Before your visit
Bring the essentials. Arrive with time for paperwork.
For a medical dermatology visit, bring your completed or updated patient forms, insurance card, photo ID, and current medication list.
Appointment forms

Arrival time
Plan to arrive 20 minutes early if you are new, have not been seen in the last three years, or have insurance or demographic changes. Established patients without recent changes should arrive at least 10 minutes early.
